Enrollment Information and Forms
New student enrollment dates for the 2012-2013 school year are August 6th, 7th & 8th, 2012.
School begins Tuesday, September 4th, 2012.
Parents enrolling their children may wish to print both the Enrollment Information Brochure and the enrollment forms below. The brochure contains information helpful to enrollment and orientation regarding school attendance areas (including streets and addresses on Fort Wood), school addresses and phone numbers, in-district transfer policies, medical information, kindergarten and first grade entry requirements, school calendar information, etc.
All of the enrollment forms must be filled out for each enrolling student, with the possible exception of the Vision form at the bottom of this page. Parents and guardians are encouraged to fill these forms out as completely as possible but wait to sign the forms, however, until present in-person at the office of enrollment. Any questions regarding particular forms will be answered at that time.
